Before commencing this application, please have the following items ready for upload:
1. Evidence
of income
Evidence of income of Parent 1 and Parent 2 (if applicable). This can be provided by way of Payment summary or Business tax return or your last 8 weeks of payslips.
2. Evidence of financial support
If available, a copy of your Government Issued Health Care Card or eligibility for receiving Family Tax Benefit Part A.
3. A short video
of your child
A short video of your child that will help us get to know them. You might like to focus on their creativity, their problem-solving capacity or how they contribute to, and take care of others and/or the environment.
